• Choose either Ten (10) BD Durasafe Plus™ CSE Trays (with either 17 or 18 gauge BD Epidural Needles), or
  
• Ten (10) BD Durasafe Plus™ Mini Kits (with either 17 or 18 gauge BD Epidural Needles)

Receive Product*
BD will deliver your FREE product. Ten (10) evaluation forms and business reply envelopes are already included in this BD Durasafe Plus™ Epidural Lock CSE Needle Evaluation Kit.

Conduct Evaluation
You and /or another participating anesthesiologist fill in an evaluation form after each procedure conducted. The
evaluation is limited to ten (10) BD Durasafe Plus™ Trays or Mini Kits per institution.

Return Form
Return each evaluation form to BD in a postage-paid business reply envelope after each procedure. BD will collate your results and return them to you for your review. To complete form online please go to www.bd.com/durasafe.
