The structure of BD infection Control Concept Cascade provides the idea: The BD vision encourage us to face the two challenges (HCWs Safety and HAIs )，which can be deal with the three methods as solution to approach，using the PDCA process as implementation tool。Continuous improvement to get the five major benefit: Improve patient outcome、Enhance reputation、Avoid Cost，Support healthcare worker safety and Meet hospital accreditation。Aim the goal: Zero tolerance to Infection!

挑戰一: 病患及醫護人員安全 Patient and Healthcare Worker Safety

BD從1988年以來一直是安全針具的先驅及領導者，保護醫護人員及病患減少針扎及免於暴露在血液感染的危險。我們引以為榮的是依臨床作業需求所做的產品設計。例如無針式設計的Q-Syte產品，簡化了輸液治療程序以減少潛在血流感染的可能。

儘管美國健康醫療體系已大多轉用安全性針具導管及採血用具，在北美以外的國家及區域仍有加強醫護人員安全的需要，BD將運用其專業知識及創新產品組合協助加強醫護人員安全。

BD has been a pioneer and world leader since 1988 in developing safety-engineered needle devices designed to protect healthcare workers and patients from exposures to bloodborne pathogens. The Company prides itself on its ability to design products that reflect an intimate knowledge of clinical processes and a deep understanding of customers’ needs. For example, the safety-engineered BD Nexiva™ Closed IV Catheter System with BD Q-Syte™ Luer Access is designed to help simplify the intravenous therapy process and reduce the potential for bloodstream infections that can be introduced through IV therapy. While the U.S. healthcare system has largely transitioned to safety-engineered syringes, catheters and blood collection devices, the need to enhance healthcare worker safety still remains outside North America. BD is well positioned to help address this need with its expertise and innovative product portfolio.

挑戰二: 醫療照護相關感染 Healthcare-Associated Infections (HAIs)

美國疾管局估計每年約有170萬的醫療照護相關感染並造成99,000人死亡，醫療照護相關感染高居死亡原因第四位;不僅如此，醫療照護相關感染也對全球醫療系統造成財務上的壓力。

想要減少日漸增加的醫療照護相關感染，關鍵之一是主動監控入院病患; 藉由快速可靠的診斷試驗，廣泛篩檢入院病患是否帶有危險的抗藥菌株，可以有效減少院內感染的發生。為此，BD提供快速的分生檢驗方法以迅速偵測諸如MRSA等多種院內感染菌; 由於在兩小時內可以獲得檢驗結果，此項快速分生檢驗方法，可以幫助臨床預防這類致命及耗費不貲的院內感染菌的散播。在歐, 亞, 美, 加等地已有250家以上醫院使用。

BD並和醫療專業團體合作，以喚起注意及提醒醫療院所關注主動監測對病患及整體經濟效益的幫助。許多世界上領先的醫療院所已認知此事。美國榮民健康協會已建議轄下的153家醫院，運用此快速分生方法，全面檢測入院病患，英國及德國並展開國家級的MRSA減少計畫。

The Centers for Disease Control and Prevention estimates there are 1.7 million infections resulting in approximately 99,000 deaths annually in the United States, making healthcare-associated infections the fourth leading cause of death. Beyond the human toll, there is an enormous financial burden to healthcare systems worldwide.

One key to reversing the rising incidence of healthcare-associated infections (HAIs) is active surveillance of patients entering healthcare facilities, which requires a diagnostic test with the ability to screen broad patient populations for the presence of dangerous organisms and rapidly deliver reliable, actionable results. To respond to this need, BD offers molecular diagnostic tests for swift, accurate detection of MRSA (methicillin-resistant Staphylococcus aureus). This technology, which produces results in less than two hours, offers BD customers a valuable tool to help prevent the spread of these potentially deadly and costly HAIs. The customer base for the BD GeneOhm™ MRSA assay has grown to more than 250 hospitals in the U.S., Canada, Europe and Asia-Pacific.

BD is collaborating with medical professional societies to build awareness and educate healthcare providers about the patient and economic benefits of using active surveillance to prevent the spread of HAIs. Many of the world’s leading healthcare institutions and networks have taken note. The U.S. Veterans Health Administration now recommends rapid molecular testing for all incoming patients at its 153 hospitals, and both the U.K. and Germany have initiated national MRSA reduction programs.

The three pillars：Best Practice, Innovative solutions and Human Compliance are the key elements of the BD IC Concept, to ensure significant impact on the areas ( 5 major items) of concern of the different stakeholders in the hospital.

Innovative Solutions are not used if they are not included in proper procedures (best practice). Innovative solutions together with best practice will still not make a difference, if people are not strictly following the best practice procedures. Therefore the BD IC concepts emphasizes on getting human compliance.

隨著個人對於自我健康意識的提昇, 感染管控的概念也將在醫療過程中越來越受到重視, 進而蔚為風潮; 在醫院裏, 醫護人員盡心盡力於照顧病患之際, 應該值得藉由更安全工具的使用, 使其免於意外的針扎感染; 進而更安心地提供病人全方位的照護, 並進而提高醫院卓越的聲譽; 隨著健保局推行疾病診斷關聯群的政策之後, 未來對於疾病的治療, 將更著重於病人從入院到出院, 甚至是出院後十四天內整個流程的控管, 目的在於減少醫療成本的支出, 進而增加醫院營運的收入; 同時, 院內感染將成為院內醫療成本的沉重負擔, 因此, 如何有效控管院內感染, 將成為醫院營運的顯學.

Infection control, which accompanied with increasing health care awareness, has been an important role of health care process. Furthermore, infection control has been an important trend during healthcare providing no matter in hospital or out. Healthcare workers should be protected from needle stick injury by using safety tools when they pay great effort on taking care of patients in hospital. By using safety tools, health workers can take of patients more concentrative, and then contribute to excellent reputation of hospital. After execution of diagnosis related group payment policy (DRGs), health care process will pay more attention on infection control not just in hospital but period after out of hospital in order to control the cost of whole health care process. In another word, healthcare associated infection control (HAIs) will be a critical burden of hospital execution. Thus, how to control HAIs will be an important lessens for hospital management.
