CareFusion may collect personally identifiable information ("PII") and non-personal information through this Site. We may use cookies, web beacons, pixel tags, log files, or other technologies to collect certain non-personal information regarding usage of the Site, including your browser type, operating system, Internet Protocol ("IP") address, the domain name from which you access the Site, and information about your browsing behavior such as the date and time you visit the Site, the amount of time you spend viewing or using the Site, the number of times you return to the Site, or other click-stream or Site data usage. We may combine this non-personal information with PII we collect from you.
CareFusion may collect certain PII when you use the Site, including your name, email address, and/or facility name ("Provider Information"). The identification of your facility name is optional. To create patient reminders, you must select the method of delivery (i.e., text message, voice message, and/or email) and provide your patient's mobile phone number, residential phone number, and/or email address ("Patient Information"). You must only use the service if your patient has provided his or her express consent to receive such reminders. Please consult our Terms of Use for Provider's representations and warranties regarding consumer consent.
CareFusion will collect Provider Information and Patient Information as necessary for the successful delivery of preoperative patient reminders. Patients' names or information related to their medical history or impending surgical procedure will not be collected on this Site
CareFusion uses the information collected through the Site for the proper administration and management of the StartClean Reminder Program. CareFusion will use the Provider Information and Patient Information to generate and deliver patient reminders via the selected delivery method(s) on the date and time requested. The reminder message will not include personalized Provider Information or Patient Information, except for email reminder messages which may identify your facility name if such information has been provided by you.
Care Fusion may contract with third parties that perform support services for the Site, including third parties that provide hosting, software development and database management services (each a "Service Provider"). Service Providers are required to maintain the confidentiality of Provider Information or Patient Information and use that information solely for the proper administration and management of this Site.
CareFusion will not share or sell Provider Information or Patient Information except as otherwise described herein. CareFusion may share the information we collect on the Site: (1) to our Service Providers, if the disclosure will enable them to perform a business, professional, or technical support function for us; (2) as necessary if we believe that there has been a violation of our Terms of Use or of our rights or the rights of any third party; and (3) to provide information to law enforcement or regulatory agencies or in connection with an investigation on matters related to public safety, as permitted by law, or otherwise as required by law. Except as described in this Privacy Statement, we will not share, sell, rent, or lease your PII to a third party for its own use.
CareFusion may share aggregate or anonymous non-personal information with third parties.
Also, CareFusion may sell or purchase assets during the normal course of our business. If another entity acquires us or any of our assets, information we have collected about you may be transferred to such entity. In addition, if any bankruptcy or reorganization proceeding is brought by or against us, such information may be considered an asset of ours and may be sold or transferred to third parties. Should such a sale or transfer occur, we will use reasonable efforts to try to require that the transferee use PII provided through this Site in a manner that is consistent with this Privacy Statement.
CareFusion is committed to safeguarding the Provider Information and Patient Information collected on this Site. CareFusion has taken reasonable and appropriate measures to manage the security of such information on this Site. However, we cannot guarantee the security of any information that is disclosed online.
