Q.

Why are we recommending using Shortage Manager for COVID-19 medications?

A.

  • Although the Shortage Manager is primarily intended for managing actual drug shortages, you can create shortage cases for any medication in your formulary.
  • Medication use is typically evaluated based on an average usage in the past 120 days and will not reflect an acute change in usage volatility.

    • The Shortage Manager uses metrics based on the last seven days of data, permitting you to more accurately forecast inventory needs based on current average daily usage.
    • If there are medications with the similar therapeutic use, assigning them as Substitutes enables you to monitor the inventory days on-hand of the whole group of medications.
    • The shortage manager permits organization of shortage cases by priority, which makes it easier to locate and monitor the cases with highest priority.
Q.

How do I manage medications in the Shortage Manager?

A.

  • To use this tool to monitor your usage of critical COVID-19 medications, create a shortage case for each medication. The case will permit you to see where that medication currently is stored, where it is (and is not) being used, days on hand, and estimated stock-out date.
  • When you experience a medication shortage at a facility, you may want to know how much of the medication the facility has, where it is located, and how fast the medication is being used. The Shortages page allows you to view and monitor pre- and post-shortage data.
  • Once you determine a medication that you want to monitor, you can create a Shortage case.
  • If you choose, you can adjust PAR levels from within the Shortage case or set them back to pre-shortage levels when the Shortage case is closed. The Shortages page provides two views:

    • Health System Shortages (a consolidated view of medication shortages across all BD HSIO enabled facilities within the Health System).
    • Facility Shortages (a view of medication shortages associated with the currently selected BD HSIO enabled facility).
  • If you are managing these drugs for an enterprise in multiple EMR systems, usage of the same medications across the enterprise may be viewed from a single point. A connection through BD HealthSight Data Manager with formulary mapping to the BD Library permits you to monitor drug usage with multiple medication identifiers (Med IDs) across your enterprise.

Q.

Will I still see PAR recommendations for these medications?

A.

No, you will proactively manage PARs within the Shortage Manager. By default, the list of shortage medications is sorted by estimated stock-out date in chronological order. Furthermore, the estimated stock-out date is displayed in red when it is less than or equal to seven days from the current date for active cases.

Q.

If I increase my PAR levels, will I see an increase in expired inventory after usage returns to previous volume?

A.

Whenever you increase inventory, there is a risk of items expiring before use. To avoid this risk, use the inventory snapshot feature of the Shortage Manager to capture pre-emergency PAR levels, and restore those PAR levels when usage returns to normal.

Q.

How can I prepare for increasing demand?

A.

Preparing Patient Care Area devices for increased inventory amounts done by performing the following:

  • Determine the desired inventory level
  • Remove items from care area devices that are not being used to create room for the additional inventory
  • Where needed, replace pockets that are near capacity with larger pockets that can hold the desired amount of inventory, or place additional pockets in the freed-up space. Pocket type is on the Medication Details page.

Q.

Won’t my inventory costs go up if I have higher maximum PAR levels?

A.

During the pandemic, it is of utmost importance to ensure that needed medications are available, while reducing trips to Patient Care areas to avert potential exposure. This means that your inventory cost will temporarily increase. Use the inventory snapshot to reduce inventory when usage returns to normal to relieve that inventory.

View the inventory valuation on the Inventory page across the enterprise as well as by facility and export to a .CSV. Filter by type to view just selected mediations such as Shortages and Substitutes.

Q.

How does the Soon-to-Expire report help me to extend dating?

A.

  • The Soon to Expire Items report looks forward in time to identify medications, in BD Pyxis™ MedStation ES devices, that are at risk of expiring in the near future.
  • The report also identifies devices with current utilization where the medication is more likely to be consumed prior to the expiration date.
  • Identified medications that are not likely to be consumed prior to their expiration date may be eligible for extended expiration dates.

    • The FDA has a list of manufacturers that have applied for drug extensions on its website located here.
