CLABSI risk and the role of best practices

In port access, compliance with best practice is critical

When it comes to port access, every step matters. Central-line bloodstream infections (CLABSIs) remain a significant risk, with as many as 250,000 CLABSIs occurring annually in the U.S.¹ Of the patients who develop a CLABSI, as many as 1 in 4 may not survive.²

To address this risk, the Infusion Nurses Society (INS) recommends following a standard Aseptic Non-Touch Technique (ANTT®) designed to help reduce the likelihood of infections.

INS Standards also note the use of single-use kits and barriers to help establish an aseptic field.

      Aligning practice
to guidelines

All-in-one port access designed to promote best practices

BD® AllPoints™ Port Access Systems bring together the key components needed to help support aseptic technique and promote consistency with port access guidelines.

Each kit is sterile, organized in procedural sequence and designed to aid clinicians in following ANTT® and INS standards across teams and shifts. Procedural checklists are included to help guide consistent technique and support standardization in practice.

How AllPoints™ Kits support best practices

Each item within the all-in-one, step-by-step AllPoints™ Port Access System is designed to promote compliance with healthcare best practices and guidelines for port access.

      How AllPoints™ Systems align

Wear…a mask (both clinician and patient) to reduce the risk of droplet transmission of oropharyngeal flora.1

Borrowing from INS Standard 27, two sterile masks (one for the clinician and one for the patient)

      Description

      How AllPoints™ Systems align

Use an alcohol-based hand rub (ABHR) containing at least 60% ethanol or 70% isopropyl alcohol routinely for hand hygiene.1

Ensure that supplies necessary for adherence to hand hygiene are readily accessible in all areas where patient care is being delivered.1

Purell® Advanced Hand Sanitizer gel packs

      Description

      How AllPoints™ Systems align

Don sterile gloves to palpate or locate port site after skin antisepsis and prior to insertion of non-coring needle.1

Two sets of sterile gloves (one for preparation and one for access)

      Description

      How AllPoints™ Systems align

Critical Aseptic Field: A large sterile drape/barrier. Used to ensure asepsis; all procedure equipment is placed upon the drape and protects multiple and often large Key-Parts collectively.

Full-size sterile field drape unfolds into a sequential layout to enable the creation of an aseptic field.

      Description

      How AllPoints™ Systems align

Perform skin antisepsis prior to port access and allow skin antiseptic agent to fully dry prior to port access.

BD® ChloraPrep™ Patient Preoperative Skin Preparation with Sterile Solution™

      Description

      How AllPoints™ Systems align

Use a single-use applicator containing an antiseptic solution.

BD® ChloraPrep™ Patient Preoperative Skin Preparation with Sterile Solution™

      Description

      How AllPoints™ Systems align

Flush and lock the port to assess function and maintain patency.
      1. Use at least 10 mL of 0.9% sodium chloride (adult).
      2. Use of 0.9% sodium chloride alone may be as effective as heparin in maintaining patency.

Sterile field-ready, pre-filled flush syringe with peel-away packaging

      Description

      How AllPoints™ Systems align

Use commercially manufactured prefilled flush syringes (when available) to reduce the risk of catheter-associated bloodstream infection (CABSI) and device failure, save time for syringe preparation, and aid optimal flushing technique and objectives.

Sterile field-ready, pre-filled flush syringe with peel-away packaging

      Description

      How AllPoints™ Systems align

Access the port with the smallest-gauge non-coring needle to accommodate the prescribed therapy; use of a safety engineered non-coring needle is recommended and required in some jurisdictions.

Confirm that a port and the non-coring needle are indicated for power injection before using it for this purpose.

Non-coring needles are available in a wide variety of gauge sizes for use with implanted ports.

For ports verified as power-injectable, BD® PowerLoc™ and PowerLoc™ Max Safety Power Injectable Infusion Sets are power injection–rated for optimal protection.

      Description

      How AllPoints™ Systems align

Consider use of a needle insertion assistive device, which may improve first-attempt success with insertion of the non-coring needle into the port.

BD® Amanda™ Port Stabilizer is designed to aid access when inserting the infusion needle into single-port devices.

      Description

      How AllPoints™ Systems align

Consider chlorhexidine-containing dressings in adults and patients over 2 months of age. Guidelines for oncology patients suggest use of a chlorhexidine- containing dressing around the needle insertion site based on duration of infusions exceeding 4 to 6 hours.

Configurations include additional CHG disc and gel pad options, including the BD® GuardIVa™ Antimicrobial Hemostatic IV Dressing.

      Description

      How AllPoints™ Systems align

Prepare skin for optimal skin health and dressing adherence.

      Sterile skin-protectant prep pad

      Description

      How AllPoints™ Systems align

Use a transparent semipermeable membrane (TSM) dressing that covers the non-coring needle and access site when the port is accessed.

Secure the non-coring needle to reduce the risk for needle dislodgement and subsequent risk for infiltration/extravasation (eg, sterile tape strips), assuring protection of skin integrity around the insertion site.

      BD® Sentrinex™ 3D Port Dressing

      Description

      How AllPoints™ Systems align

A sterile dressing, combined or integrated with a securement device appropriate for patient’s condition and patient preference, is maintained on all peripheral and central VADs to protect the site, provide a microbial barrier, and promote skin health and VAD securement.
